    In order for High School students to be eligible to start their season, they must have 5 items turned into the athletic office. If you are not sure of your last physical or impact date, please check now. Physical appointments can be difficult to schedule, so please don’t wait until the last minute. All forms are available online via the High School website under Student Life or available in the High School main office.  Students will not be able to practice until ALL five items are received. The five required items are: 
    1. WIAA Physical or Alternate Year card
    2. Current IMPACT Testing
    3. Concussion Waiver
    4. Athletic Fee Payment of $75
    5. Code of Conduct Video & Acknowledgement Form - please click on the WIAA Code of Conduct Requirement in the left column of this page to complete

    Impact testing dates will be offered in a couple of sessions prior to the Winter seasons begin.  Watch for updated website postings, announcements at school and an email to be sent with those dates. Testing is done for free at the Middle School/High School and is required every other year. If you have questions about the Impact testing, please contact Sheri Almquist at almquistsheri@mhasd.k12.wi.us.  **Student athletes who are in need of impact testing for their Fall sport eligibility should contact UW Health Sports Medicine at 608.265.8886 to schedule at an appointment to complete this required item.  The five sessions offered through the school concluded on August 6th.  

    Winter Athlete Impact Testing Dates:

    October 30th, 5pm at Middle School Computer Lab

    November 7th, 3:45pm at Middle School Computer Lab

    The Code of Conduct video is now available online. The code video must be viewed by the student and parent. As students enjoy their summer months off, please remember that our Code of Conduct rules are in effect for all 12 months of the year and participation in extracurricular activities is a privilege, not a right.
